Funding Applications Open Until March 10, 2023
Funding will be provided to 75 families, who have children with a diagnosis of cerebral palsy, in the amount of up to $500 to use for therapy. Families must reside in Ontario. Therapists must be located in Ontario.
Applications are available on our funding page. Applications will be accepted until Friday, March 10, 2023, 11:59pm or earlier if 75 complete applications are received.
NEW THIS YEAR - Applications will be completed online using Google Forms. You do not need to have a Google account to complete the application.
Received funding from us in the past? Simply complete the application! We have your supporting documentation.
Haven't received funding from us the in past? You must submit supporting documentation in a separate application. NEW THIS YEAR - You can submit the supporting documentation in the separate application using your Google account, or, you can send the supporting documentation by email to info@noahsclubhouse.org if you do not have a Google account.
Funding will be distributed based on the order of complete applications received.
Applications are considered complete when both application AND supporting documentation, if required, are received. It is the applicant's responsibility to ensure the application and any supporting documentation are received and complete.
Supporting documentation to be included: 1. medical documentation for diagnosis of cerebral palsy, AND, 2. documentation from practitioner prescribing therapy.
Families will receive an email confirming receipt of the applications within 48 hours.
Notifications of acceptance will be emailed by April 30, 2023.
Funding must be used by December 31, 2023. There are NO extensions.
