FUNDING ANNOUNCEMENT

We are growing once again!! We are thrilled to announce we will be providing funding to 75 families for the first time!!

Funding Applications Open From Saturday, March 4 to Friday, March 10, 2023

• Funding will be provided to 75 families, who have children with a diagnosis of cerebral palsy, in the amount of up to $500 to use for therapy. Families must reside in Ontario. Therapists must be located in Ontario.

• Applications will be available starting on Saturday, March 4, 2023 at 8:00am on our funding page of our website as well as Noah's Clubhouse Facebook page. Applications will be accepted until Friday, March 10 2023, 11:59pm.

• NEW THIS YEAR - Applications will be completed online using Google Forms. You do not need to have a Google account to complete the application.

• Families who have received funding from Noah's Clubhouse in prior years need to only complete the application as the supporting documentation has been received in the past.

• For families who have not received funding in the past from Noah's Clubhouse, you will need to provide supporting documentation in a separate application. NEW THIS YEAR - you will have the option of uploading the supporting documentation in the separate application using your Google account, or, sending the supporting documentation by email to info@noahsclubhouse.org if you do not have a Google account.

• Please note, for the families submitting supporting documentation, your application will be considered complete once both the application and supporting documentation are received.

• Supporting documentation to be included:

1. Medical documentation for diagnosis of cerebral palsy, AND,

2. documentation from practitioner prescribing therapy.

• Funding will be distributed based on the order of complete applications received. It is the applicant's responsibility to ensure the application has been received and is complete.

• Funding must be used by December 31, 2023.

• An email confirming receipt of your supporting documentation will be sent within 48 hours.

• Notifications of acceptance will be emailed by April 30, 2023.
